WAIVER OF LIABILITY

The undersigned hereby give permission for the child(ren) noted above 'PLAYER' to participate in the athletic program noted above sponsored by the Crofton Athletic Council (CAC). It is understood that the CAC is a nonprofit, volunteer organization which sponsors the above noted sport programs as a community service, and it is not responsible for expenses or damages resulting from injury to participants or spectators which is sustained in conjunction with or incidental to the above CAC sanctioned activity.

Accordingly, the undersigned HEREBY EXPRESSLY AGREE to waive all claims against, and hold exempt from liability, the CAC, its officers, directors, coaches, and any other person affiliated with the Association for injury or injuries sustained by the above referenced child(ren), from whatever cause, while attending, participating in, or traveling to or from Association activities. IT IS FURTHER AGREED that the undersigned hereby waive any and all claims against the Division of Amateur Athletics of the Department of Recreation and Parks (Rec & Parks) of Anne Arundel County, its supervisors, managers, or any other person affiliated with the above sport(s) should that sport(s) by a sanctioned CAC activity and under the auspices of (rec & parks), for injury or injuries sustained while watching or playing games or traveling to and from games.
